Challenge

A major office supply retailer aimed to perform a lockout tagout (LOTO) kaizen followed by a LOTO assessment across four fulfillment centers. The objective was to create equipment-specific LOTO procedures for each piece of equipment used in these facilities.

Kaizen is a Japanese term meaning continuous improvement or change for the better.

Antea Group contributed LOTO expertise through a one-day kaizen event at one of the client's fulfillment centers and conducted walkthroughs at all four facilities.
 

Solution

Antea Group facilitated the kaizen event where environment, health, and safety (EHS) and facility staff worked to identify potential gaps in the company’s LOTO policies and procedures. Brainstorming sessions occurred throughout the day to discuss current practices and potential improvements that could be rolled out systemwide. Following the kaizen, the client requested that Antea Group conduct follow-up facility assessments.

The goal of the four on-site assessments was to aid in the development of equipment-specific LOTO procedures. The assessments were designed to review the machinery and equipment on which company employees perform maintenance activities and document steps to safely lock out hazardous energy sources prior to commencing work.

During each assessment, a facility walkthrough was conducted with members of the facility staff to identify each piece of equipment that requires client employees to conduct maintenance or repair. Detailed notes on operation, shutdown, and power sources along with photographs were collected for each piece of equipment.

Fulfillment centers were visited in four states in various parts of the United States where approximately 30 unique pieces of equipment were identified for equipment-specific LOTO procedures.

Result

Antea Group developed detailed equipment-specific LOTO procedures for all the identified equipment. The procedures included steps for each part of the process of isolating all energy sources; photographs for each step to point out switches, buttons, valves, etc.; and steps for the verification of energy isolation.

These equipment-specific procedures were rolled out to all the client’s locations in conjunction with LOTO training developed by Antea Group.
